We are seeking a highly skilled Dietitian to join our team as a Dietitian Care Specialist. As a valued member of our healthcare team, you will provide exceptional nutrition care and education to PACE participants.

Key Responsibilities:

  • Evaluate participants' nutritional status and develop individualized nutrition care plans in collaboration with the interdisciplinary team.
  • Assess nutrient requirements, recommend dietary prescriptions, and monitor their effectiveness while ensuring compliance with PACE regulations and standards of care.
  • Provide nutrition education and counseling to participants and their caregivers, aligning with the plan of care.
  • Maintain thorough documentation of nutritional assessments, care plans, and interventions in participants' medical records.
  • Work with participants to prevent malnutrition, promote weight management, and improve overall health outcomes through targeted nutrition strategies.

Requirements:

  • Bachelor's degree in Nutrition or Dietetics from an accredited institution.
  • Current registration from the Commission on Dietetic Registration (CDR).
  • Minimum one year of experience working with frail and/or elderly populations, preferably in a healthcare or PACE setting.

Salary Estimate: $65,000 - $85,000 per year, based on experience and location.
