MDS Coordinator Job Description

Rose Villa Healthcare Center is seeking a highly skilled and dedicated MDS Coordinator to join our team. As an MDS Coordinator, you will play a critical role in ensuring the timely completion of the Resident Assessment Instrument (RAI), the Minimum Data Set (MDS), Care Area Assessments (CAAs), and Care Plans in accordance with State and Federal requirements.

Key Responsibilities:
  • Coordinate the interdisciplinary team in the timely completion of assessments.
  • Utilize MDS Scheduler to plan and set MDS Assessment Reference Dates (ARDs).
  • Communicate ARD selection for specific MDSs to the IDT.
  • Complete MDS sections as assigned.
  • Complete CAAs as required.
  • Complete Care Plan items and attend Care Plan meetings as assigned.
  • Monitor overall documentation in the medical record to validate that it supports MDS coding.
  • Participate in skilled utilization and State Case Mix management as assigned.
  • Provide teaching and training for MDS item completion to interdisciplinary team members.
  • Provide teaching and mentoring of CNA staff related to the completion of the Nursing ADL documentation.
Requirements:
  • LVN, LPN, or RN licensure.
  • At least two years of experience working in a long-term care setting.
  • Experience with RAI process and MDS certification preferred.
