Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team at the Meadows of Empire Care Centers. As an LPN, you will play a vital role in providing high-quality care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring
  • Develop and implement individualized care plans, taking into account the unique needs and preferences of each resident
  • Collaborate with the healthcare team to ensure seamless transitions of care and effective communication
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures
Requirements
  • Current LPN licensure in the state of Georgia
  • 1+ year of experience in a long-term care setting
  • Ability to work effectively in a fast-paced environment and prioritize tasks accordingly
  • Strong communication and interpersonal skills, with the ability to work collaboratively with the healthcare team
