The Care Coordinator RN will function as an integral member of the household clinical team, planning and supporting person-directed living according to physician orders, nursing orders, care plans, and villager choice, focusing on both medical and social needs to maximize quality of life.

The estimated annual salary for this position is around $80,000-$100,000, depending on experience. The additional shift premiums range from $3 to $5 per hour, providing a total compensation package that reflects the importance of this role.

Key Responsibilities:

  • Develop and implement personalized care plans in collaboration with the interdisciplinary team.
  • Assess villager's medical and social needs, organizing notes, gathering documents, and assisting MD during acute care rounds.
  • Report and document changes in condition according to policy.
  • Identify and report infections to Infection Control/Skin Care Coordinator and set up necessary precautions.
  • Serve as a supervisory resource for the Neighborhood Manager and Registered Nurse Specialist, directing and evaluating the performance of Nursing Assistants.
  • Edit nursing team members and household team members on policies and procedures.
  • Attend nurse meetings and neighborhood/household meetings, ensuring participation in Quality Improvement and follow-up to ensure quality outcomes.
  • Promote empowerment of the team in providing person-directed living, leading and mentoring the household, and serving as a role model.
  • Maintain a safe, comfortable, and functional environment by assessing the household for potential hazards and implementing practices regarding sanitation, infection control, fire safety, disaster preparedness, and emergency care.
