Job Title: Advanced Practice Provider

We are seeking an experienced Advanced Practice Provider to join our Community Health/Primary Care float team at JPS Health Network and Acclaim Physician Group.

The float team supports 11 JPS Patient Centered Medical Homes throughout Tarrant County, comprised of fantastic teams of Family Medicine, Internal Medicine, Pediatric, and Advanced Practice Professionals.

Job Summary:

Working under the supervision of a physician, the Advanced Practice Provider is responsible for administering and interpreting all aspects of patient care, including diagnosis, treatments and consultations within the outpatient setting.

This position will also educate patients about preventative care and prescribed treatments; may also conduct physicals, order tests and serve as a patient's healthcare provider.


Duties and Essential Job Functions:

  • Collaborates with the physician in overseeing and coordinating the multidisciplinary care delivered to all patients, to optimize quality patient care.
  • Reviews patient medical charts to assist physician with diagnosis, treatment and care management.
  • Makes appropriate patient referrals as necessary.
  • Monitors guidelines for the management of care and for ensuring that the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.
  • Provides care education to patients and families.
  • Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores.
  • Maintains a collaborative environment with providers, clinical staff, and support staff.
  • Supports academic, educational and research endeavors.
  • Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.
  • Serves as a resource to the community.
  • Performs other job related duties as assigned.

Qualifications:

Required Education and Experience:
Masters Degree in Nursing from an accredited school of nursing OR Graduate of an accredited Physician Assistant Program
Minimum of 2 years working independently as an advanced practice provider in a primary care setting

Required Licensure/Certification/Specialized Training:
Must have current Texas Physician Assistant license or Texas APRN license with FNP certification
Valid, unrestricted DEA
Basic Life Support (BLS) certification
