  • Assess and evaluate patients' needs, developing an appropriate plan of care.
  • Provide education and strategies for patients and families.
  • Coordinate referrals to home care to promote continuity of care.
  • Collaborate with VNA clinicians and external providers to achieve best patient outcomes.
  • Document all changes in patients' conditions and update care plans and outcomes.
Qualifications:
  • RN or LPN with a valid NJ nursing license.
  • 2+ years of community health nursing experience.
  • Excellent verbal and written communication skills.
  • Strong interpersonal and organizational skills.
  • Client-focused, assertive, and creative in problem-solving.
