Job Title: Licensed Practical Nurse

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in Spokane Valley, WA and surrounding areas. This is a home health position that requires a strong commitment to patient care and a passion for delivering high-quality services.

Job Summary:

This LPN position involves providing skilled home health care to patients in their homes. The ideal candidate will have at least 2 years of experience in skilled home health and be able to work independently with minimal supervision. The position requires a strong understanding of home health care principles and the ability to work effectively with patients, families, and healthcare teams.

Responsibilities:
  • Provide skilled home health care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
  • Document patient care activities and outcomes in a timely and accurate manner.
  • Participate in ongoing education and training to stay current with best practices in home health care.
Requirements:
  • Current LPN licensure in the state of Washington.
  • At least 2 years of experience in skilled home health care.
  • Strong understanding of home health care principles and practices.
  • Ability to work independently with minimal supervision.
  • Excellent communication and interpersonal skills.
