Job Summary:

Our Occupational Therapist will work with patients in their homes, providing skilled services to those who meet the criteria for Medicare home health coverage. You will work closely with our team to develop personalized treatment plans that address the unique needs of each patient.

Responsibilities:
  • Conduct thorough assessments of patients to identify their strengths and weaknesses
  • Develop and implement individualized treatment plans to address patients' needs
  • Provide skilled services to patients in their homes, including occupational therapy interventions and education
  • Collaborate with our team to ensure seamless care coordination and communication
  • Document patient progress and outcomes in a timely and accurate manner
Requirements:
  • Active license in the state of TX required
  • OT Liability insurance in the state of TX required
  • Graduate degree or greater in Occupational Therapy
  • Home Health or Rehab experience preferred
  • Ability to manage patients with different types of personalities
  • Current knowledge of treatment practices
  • A friendly and professional demeanor is required when explaining procedures and treatments
