VNA Health Group is seeking a skilled and compassionate Registered Nurse (RN) to join our team as a Per Diem RN in our Mobile Outreach Clinic Program.

Job Summary:

We are looking for a dedicated and experienced RN to provide high-quality nursing care to patients in their homes and communities. As a Per Diem RN, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and providing education and support to patients and their families.

Responsibilities:
  • Provide skilled nursing care to patients in their homes and communities
  • Assess patient needs and develop individualized care plans
  • Implement care plans and provide education and support to patients and their families
  • Monitor and ensure quality and appropriateness of services provided
  • Develop and maintain community relationships to support client referrals
  • Adhere to agency infection control and safety policies
  • Adhere to the organization's policy in regards to absenteeism and appearance
Qualifications:
  • RN licensure for New Jersey
  • 2+ years of community health nursing experience; bilingual preferred
  • Strong computer skills
  • Excellent verbal and written communication skills
  • Excellent interpersonal skills with demonstrated ability to communicate with outside personnel
  • Ability to travel with a valid NJ driver's license required
