BMC Nurse Case Manager Professional

Welcome to the exciting opportunity of joining WVU Medicine as a skilled Nurse Case Manager. We are seeking a dedicated professional to fill this vital role in our healthcare team.
In this position, you will be responsible for comprehensively planning and managing targeted patient populations. You will perform resource management, including denial management, utilization management, access to appropriate levels of care, discharge planning, care facilitation, and referral to other levels of care. As a member of the multidisciplinary care team, you will work collaboratively to facilitate achievement of desired treatment outcomes.
To excel in this role, you will need to possess excellent communication and interpersonal skills, with the ability to work effectively with patients, families, healthcare providers, and other stakeholders. Your strong knowledge of healthcare policies, procedures, and regulations is essential for ensuring high-quality patient care and meeting regulatory requirements.
We offer a competitive salary range of $68,000 - $82,000 per year, based on experience, to support your professional growth and success in this challenging yet rewarding role.
**Key Responsibilities:**
* Manage all aspects of transition/discharge planning for assigned patients in a timely manner
* Collaborate with multidisciplinary teams to facilitate transition/discharge processes for designated caseloads
* Monitor patient progress, intervening as necessary to ensure plans of care and services provided are patient-focused, high-quality, efficient, and cost-effective
* Provide education to staff, physicians, and patients/families to ensure effective transition planning
* Meet directly with patients/families to assess needs and develop individualized transition/discharge plans in collaboration with physician teams
* Communicate complex family dynamics that may impact patient care and transition/discharge planning with multidisciplinary teams and post-acute providers when applicable
* Initiate and facilitate referrals to post-acute services, including homecare, durable medical equipment, hospice care, long-term acute care facilities, acute rehab facilities, and skilled nursing facilities
Requirements:
* Current unencumbered licensure with the WV Board of Registered Professional Nurses or licensure as a Registered Professional Nurse in another state with a temporary West Virginia practice permit, or equivalent licensure in the state where services will be provided
* Five (5) years clinical experience
* Bachelor's degree in Nursing (BSN) preferred
Working Environment:
This role requires frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping, and feeling throughout the work shift. You must be able to lift, move, and position patients of all weights, with varying amounts of assistance in a fair manner on a regular and recurring basis. Your hearing and visual acuity must be within normal ranges, with the ability to perform night driving as necessary for patient care. You will also be required to exert in excess of 100 pounds of force occasionally, and/or up to 50 pounds of force frequently, and/or up to 20 pounds of force constantly to move objects.
