Your Role:
As an MDS Coordinator, you will play a vital role in enhancing the quality of life for our residents. Your responsibilities will include:
  • Providing compassionate care and support to our residents.
  • Conducting thorough and timely assessments using RAI tools to ensure individualized care.
  • Acting as an advocate for residents by facilitating family and resident involvement in care planning.
  • Adhering to federal timelines in the RAI and care planning processes.
  • Participating in interdisciplinary care plan meetings to promote positive resident outcomes.
Qualifications:
  • Exemplifying C.A.R.E. values in all interactions.
  • Availability to work overtime as needed.
  • Graduation from an accredited School of Nursing, preferably as a Registered Nurse.
  • One to three years of experience with MDS knowledge and data entry in long-term care.
  • Two years of professional nursing experience in relevant care settings.
  • Familiarity with the Resident Assessment Instrument and interdisciplinary care planning.
  • Understanding of OBRA regulations and current state reimbursement guidelines.
  • Knowledge of laws and regulations affecting care in nursing facilities.
