Your Role:
As an MDS Coordinator, you will play a vital role in enhancing the quality of life for our residents. Your responsibilities will include:
- Providing compassionate care and support to our residents.
- Conducting thorough and timely assessments using RAI tools to ensure individualized care.
- Acting as an advocate for residents by facilitating family and resident involvement in care planning.
- Adhering to federal timelines in the RAI and care planning processes.
- Participating in interdisciplinary care plan meetings to promote positive resident outcomes.
- Exemplifying C.A.R.E. values in all interactions.
- Availability to work overtime as needed.
- Graduation from an accredited School of Nursing, preferably as a Registered Nurse.
- One to three years of experience with MDS knowledge and data entry in long-term care.
- Two years of professional nursing experience in relevant care settings.
- Familiarity with the Resident Assessment Instrument and interdisciplinary care planning.
- Understanding of OBRA regulations and current state reimbursement guidelines.
- Knowledge of laws and regulations affecting care in nursing facilities.
