We are currently seeking a Healthcare Assistant to join our Care Management department. This role involves overseeing a designated group of patients' care and assisting them in reaching their health objectives, both short-term and long-term. The Healthcare Assistant will collaborate with healthcare providers to ensure that patients receive the necessary support and care.

Key Responsibilities:

  • Oversee a patient roster enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs.
  • Serve as a primary contact between patients and healthcare providers.
  • Coordinate and confirm patient appointments.
  • Maintain a thorough understanding of each patient's care plan and ensure adherence.
  • Document patient information daily and notify healthcare providers of any concerning findings.

Qualifications:

  • Minimum of 1 year of clinical experience as a Medical Assistant.
  • At least 1 year of experience with Electronic Medical Records (EMR); familiarity with eClinicalWorks is preferred.
  • Demonstrated professionalism and strong communication skills.
  • Exceptional customer service abilities.
  • Completion of a Medical Assistant program or possession of a CMA certification is required.
