  • Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Manage patient caseloads, including scheduling appointments, coordinating services, and documenting patient information.
  • Participate in on-call duties and provide 24/7 support to patients and families.
Requirements:
  • Graduate of an accredited school of nursing with a current RN license in the state.
  • 1-2 years of recent acute care experience in an institutional setting, with a preference for home care experience.
  • Excellent observation, verbal, and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
  • Prolonged or considerable walking or standing, ability to lift, position, and/or transfer patients, and ability to lift supplies and equipment.
