Job Summary
This role involves providing nursing services across multiple health centers. The typical work schedule is Monday through Friday, from 8:30 a.m. to 5:00 p.m. A sign-on bonus may be applicable, subject to certain conditions including hours worked and experience.
The Registered Nurse (RN) for Health Centers is responsible for coordinating treatment during their shift and executing medical interventions. The RN makes treatment decisions in accordance with RN licensure regulations and provides care as prescribed by physicians and nurse practitioners, while fostering a Trauma-Informed Care environment. Given the high incidence of trauma among individuals receiving mental health services, it is essential for the RN to comprehend the impact of trauma on health and coping mechanisms.
All staff members are expected to understand, appreciate, and respect the diversity and cultural differences within our community. Awareness and sensitivity to the perspectives of those we serve, as well as our colleagues, are paramount. As a Trauma-Informed organization, it is crucial for the RN to support this environment and understand the implications of trauma on health and well-being.
Key Responsibilities

- Deliver high-quality nursing care, including all medical interventions and thorough documentation.
- Develop Nursing Care Plans for various medical and physical conditions.
- Administer PRN medications based on presenting symptoms and conduct assessments to determine necessity.
- Engage in planning, implementing, evaluating, and documenting direct care activities based on sound psychiatric principles.
- Participate actively in Treatment Team meetings.
- Serve as a liaison with medical staff, pharmacy, and laboratory services to ensure continuity of care.
- Emphasize community reintegration through a comprehensive rehabilitation program, including therapy, skills-building, and vocational training.
- Administer psychotropic medications, assess resident responses, and take appropriate actions based on current standards.
- Collaborate with team members to establish medical treatment plans and resident care.
- Interact appropriately with residents and community members to support the treatment process.
- Maintain accurate electronic health records (EHR) and ancillary documentation in compliance with regulations.
- Attend Nursing meetings and case conferences.
- Demonstrate knowledge of common medical conditions encountered in individuals with severe mental illness.
- Inform residents about their medications and treatments.
- Uphold client rights and responsibilities while maintaining professional boundaries.
- Assist in crisis situations as needed, following emergency protocols.
- Ensure the safety of residents, staff, and property, and be familiar with safety regulations.
- Perform additional duties as assigned.
- Active, unencumbered Oregon Registered Nurse License and CPR/First Aid Certification. Must demonstrate competencies for QMHA or QMHP credentialing as per MHACBO.
- QMHP: Bachelor's degree in Nursing from an accredited institution.
- QMHA: Associate's degree in Nursing from an accredited institution and at least one year of mental health experience.
- Preferred: Two years of clinical experience in inpatient or outpatient settings.
- Knowledge of medical conditions commonly faced by individuals with severe mental illness.
- Familiarity with Motivational Interviewing techniques.
- Ability to maintain a trauma-informed care environment.
- Proficient in computer operations for job functions.
- Must be able to travel to various health center locations.
- Work is conducted in an outpatient clinical setting, with potential interruptions during meal periods and breaks.
- Typical workdays may involve sitting and using computers and office equipment, with transportation to other sites as needed.
- The RN must assist in ensuring safety for all clients and staff in potentially volatile situations.
