Job Summary

We are seeking a skilled Physical Therapist to join our team at HexaQuEST Health, Inc. as an Outpatient Rehabilitation Therapist. As a key member of our rehabilitation team, you will be responsible for providing high-quality physical therapy services to patients in various settings, including outpatient clinics and skilled nursing facilities.

Key Responsibilities
  • Conduct diagnostic assessments and develop treatment plans for patients with a range of conditions, including musculoskeletal, neurological, and cardiovascular disorders.
  • Provide individualized physical therapy services, including exercises, modalities, and education, to promote patient recovery and independence.
  • Collaborate with other healthcare professionals, including physicians, occupational therapists, and speech-language pathologists, to ensure comprehensive patient care.
  • Maintain accurate and timely documentation of patient progress and treatment plans.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements
  • Master's degree in Physical Therapy from an accredited program.
  • Current licensure as a Physical Therapist in the State of Hawaii.
  • Minimum 6 months of experience in an outpatient or skilled nursing facility setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Ability to work variable shifts, including weekends and holidays, as needed.
