Position Overview:
  • Accountable for the organization, development, and management of skilled nursing services in alignment with nursing standards, state and federal guidelines, and Knute Nelson's operational policies.
  • Optimize time management to effectively utilize Knute Nelson's resources while addressing client requirements.
  • Ensure the highest level of quality care, customer satisfaction, financial accountability, and adherence to regulations within the Home Health division.
Primary Responsibilities:
  • Direct Patient Care - Implement preventative and rehabilitative nursing interventions as necessary for client safety and care. Monitor and report any changes in the client's condition to the physician, including reactions to treatments and medications. Educate, supervise, and support clients and their caregivers regarding nursing care needs and related issues at home.
  • Documentation and Billing - Maintain accurate and timely records of services provided using the agency's electronic medical record (EMR) system. Demonstrate knowledge of various payer sources and required authorizations. Complete and document supervisory visits with LPNs and/or HHAs in accordance with regulations.
  • Effective Communication - Engage in clear communication with clients, families, referral sources, physicians, and the interdisciplinary team. Exhibit strong verbal and written communication skills.
  • Additional Responsibilities - The duties outlined above are representative of the nature and level of work assigned and are not exhaustive.
Qualifications:
  • Must hold a valid Registered Nurse license in the state of Minnesota.
  • CPR certification is required within six months of hire.
  • Possession of a valid driver's license and access to an insured personal vehicle is necessary.
  • Completion and passing of a state-mandated background study is required.
  • Ability to communicate effectively in both verbal and written forms as required by the position.
  • Physical capability to perform the duties associated with the role.
  • Proficiency in reading, writing, and speaking English to effectively communicate with staff, clients, and visitors.
Reporting Structure: Reports to the Regional Clinical Manager
