Patient Care Coordinator - Virtual

We are seeking tech-savvy Certified Medical Assistants to join our team as Patient Care Coordinators. In this role, you will leverage your clinical expertise and communication skills to deliver virtual care to a panel of chronically ill patients.

Responsibilities:
  • Provide comprehensive patient support through virtual phone consultations.
  • Develop personalized care plans addressing physical, mental, and preventative health needs.
  • Coach patients virtually to achieve their care goals by adhering to medical treatment plans, including healthy lifestyle practices.
  • Connect patients with relevant resources such as nutritionists, nurses, and physicians.
  • Prepare patients for appointments and procedures, ensuring smooth transitions within the healthcare system.
  • Coordinate administrative tasks related to medical records, appointment scheduling, and insurance claims.
  • Review insurance plans and provide guidance on maximizing patient benefits.
  • Proactively suggest appropriate services or preventive screenings based on individual patient needs.
Qualifications:
  • Current Medical Assistant (MA) Certification required.
  • Minimum of two years of clinical experience in a healthcare setting.
  • Experience with Electronic Medical Records (EMR) systems.
  • Proficiency in Microsoft Office Suite, including Teams, Outlook, SharePoint, and Excel.
  • Strong communication, interpersonal, and problem-solving skills.
  • Must reside and work within the Mountain or Pacific time zone.
Work Environment:

This is a fully remote position. A reliable high-speed internet connection, a dedicated workspace free from distractions, and adherence to HIPAA privacy regulations are essential.
