Nurse Care Specialist Job Summary

The Nurse Care Specialist is a key member of our healthcare team, providing high-quality patient care by utilizing the nursing process and collaborating with inter-professional colleagues. This role requires strong assessment, education, and coordination skills to ensure seamless care for patients throughout their continuum of care.

Responsibilities:
  • Utilize the nursing process to provide individualized care to patients, focusing on patient assessment, education, and coordination of care.
  • Collaborate with physicians and other healthcare professionals to plan, implement, and evaluate care plans.
  • Demonstrate expertise in nursing practices, adhering to age and population-specific standards as designated in the clinical setting.
  • Provide preventative care, health maintenance, diagnosis, treatment, and follow-up care to patients in all phases of their care journey.
  • Plan and coordinate patient care, including patient assessment, education, triage, and various nursing interventions.
Qualifications:
  • Graduate from an accredited nursing program, such as American Association of Colleges of Nursing (AACN), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA).
  • Holds an unencumbered RN license with the State Board of Nursing where practice is occurring, or possesses multistate licensure if in a Nurse Licensure Compact (NLC) state.
  • Maintains required department-specific competencies and certifications.
