Seamless Health Journey for Patients
">Optum, a clinician-led care organization, is seeking an experienced Nurse Practitioner or Physician Assistant to join its HouseCalls team in Columbia or Greene Counties, NY. As part of the broader Home and Community Care team, you will provide annual clinical assessments to patients in the comfort of their homes.
">This critical preventive care helps identify and reduce health risks for patients, coordinating follow-up care to improve overall health and well-being. ">The role requires conducting in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams. You will also perform evidence-based practice screenings, identify diagnoses for care management, formulate lists of current and past medical conditions, communicate findings with primary care physicians, and educate patients on disease processes, medication, and compliance.
">To succeed in this position, you must possess an active NP or PA license, ANCC or AANP national certification, and the ability to complete physical requirements of the job. Reliable transportation for home visits is also essential. Preferred qualifications include experience in gerontology, cardiology, internal medicine, or endocrinology, as well as home health care or home visit experience.
">We offer a competitive salary range of $88,000 to $173,200 annually, based on factors like education, work experience, certifications, and local labor markets. Our comprehensive benefits package includes incentive and recognition programs, equity stock purchase, and 401(k) contribution.
