About the Position

We are seeking a highly skilled and experienced Certified Diabetes Care Professional to join our team at Ochsner Health. This is a challenging and rewarding role that requires strong clinical skills, excellent communication abilities, and a passion for delivering high-quality patient care.

The ideal candidate will have experience working with patients with diabetes, a strong understanding of diabetes management principles, and the ability to develop and implement effective education plans. As a Certified Diabetes Care Professional, you will work closely with patients, healthcare providers, and other members of the care team to provide comprehensive support and guidance.

Responsibilities

  • Conduct thorough assessments of patients' physical health, medications, psychosocial issues, and identify areas for education and clinical interventions based on current national diabetes care standards and guidelines.
  • Develop and implement individualized education plans to address the unique needs and goals of each patient.
  • Provide education to patients and caregivers on necessary self-care practices involved in diabetes management, including disease process, acute and chronic complications, nutrition, medications, exercise, and special self-care needs.
  • Serve as a diabetes care resource to healthcare professionals, including staff, clinicians, and the community.

Requirements

  • Bachelor's degree in Nursing or related field required; Master's degree preferred.
  • 3 years of experience working with direct patient contact required.
  • Certification as a Diabetes Care and Education Specialist (CDCES) by the Certification Board for Diabetes Care and Education Specialists (CBDCES) preferred.
