Job Summary:

">About the Role:
">As a Supportive Care Associate, you will be responsible for providing assistance to residents with daily activities, treatments, and emotional health needs. You will also contribute to the development and revision of service plans by sharing your knowledge of resident preferences and successful approaches.

">Responsibilities:
">">* Assist residents with daily activities, treatments, and emotional health needs as indicated in service plans
">* Inform supervisors of current resident needs and preferences, as well as successful service approaches to develop and revise service plans
">* Report resident problems to LPNs or Medication Technicians/Aids
">* Attend training sessions and team meetings as requested
">* Perform other duties assigned by the Wellness Director

">Requirements:
">* Must be at least 18 years old
">* Demonstrate effective communication skills in English, both verbally and in writing
">* Maintain any required certifications as per state or provincial regulations
">* Meet state or provincial health-related requirements
