Job Summary:
This role requires a skilled nurse practitioner to work collaboratively with a multidisciplinary healthcare team in providing high-quality patient care. You will manage a distinct patient population, perform coordination of outpatient and inpatient care, and facilitate discharges/outpatient follow-up. Your strong foundation in evidence-based practice and education will ensure adherence to clinical pathways/protocols and promote an environment of safe quality care.
Key Responsibilities:
• Assessment of Health Status:
• Perform comprehensive history and physical examinations for acute and complex chronically ill patients, documenting key findings in the medical record.
• Order and collect relevant data using assessment techniques, diagnostic information, and procedures as indicated.
• Serve as a first responder for nursing and house staff in situations requiring procedural and resuscitative interventions.
• Assess risks associated with patient care, including medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions, and diagnostic procedures.
• Diagnosis:
• Formulate differential diagnoses by priority, managing diagnostic tests through ordering, interpretation, performance, and supervision.
• Implement and modify individualized plans of care to address complex acute and chronic health care needs, utilizing evidenced-based practice guidelines or protocols.
• Prescribe diagnostic strategies and therapeutic interventions, both pharmacologic and non-pharmacologic, to achieve expected outcomes.
• Manage further diagnostic tests and implement advanced procedures consistent with privileges and competency validation.
Communication and Collaboration:
• Maintain ongoing communication and collaboration with interdisciplinary healthcare teams, facilitating patient-centered care across the continuum.
• Document patient progress via daily progress notes, transfer summaries, discharge summaries, and/or clinic notes, ensuring timely, compliant documentation that captures patient acuity.
Professional Practice:
• Demonstrate professional practice behaviors, including preceptorship, education, and mentoring of students, nursing staff, nurses, graduate, and novice nurse practitioners.
• Seek opportunities for active engagement in research and analysis of evidence-based practice, contributing to Grand Rounds, APN council meetings, faculty meetings, and M&M presentations.
• Maintain CEUs and membership in professional organizations, enhancing professional development and growth.
