Key Responsibilities:

  • Provide physical therapy services to patients in their homes, as ordered by the hospice physician
  • Conduct assessments to develop individualized plans of care and make adjustments as needed
  • Treat patients to relieve pain, maintain function, and promote comfort using physical means
  • Counsel patients and families on disease management and provide education on proper use of equipment
  • Collaborate with the interdisciplinary team to ensure comprehensive care

Requirements:

  • Current license to practice as a Physical Therapist in the state of employment
  • Minimum of one year of experience as a Physical Therapist
  • Home care or Hospice experience preferred
  • Ability to effectively communicate and create positive impressions with patients, families, and colleagues

Essential Functions:

  • Drive to patient locations per scheduled visits
  • Document all aspects of visits within 24 hours
  • Complete and submit required documentation within specified company requirements
