Key Responsibilities- Provide clinical coverage for primary Advanced Practice Clinicians during periods of absence
- Manage members with acute medical situations or exacerbations of chronic conditions, including testing, diagnostics, and interventions
- Conduct comprehensive assessments and develop effective care plans
- Communicate and collaborate with interdisciplinary care teams
- Ensure accurate and complete documentation, including ICD-10 conditions
- Active ANCC or AANP national certification in Family, Adult, or Adult-Gerontology Primary Care
- Current, active DEA licensure or eligibility to obtain post-hire
- 1+ years of clinical experience as a Nurse Practitioner
- Willingness to travel and obtain additional licensure in assigned geographic areas
- Ability to complete physical requirements of the job
- Experience in meeting the medical needs of patients with complex behavioral, social, and/or functional needs
- Understanding of Geriatrics and Chronic Illness
