Key Responsibilities
  • Conduct Comprehensive Assessments: Conduct thorough assessments of patients' physical, psychosocial, emotional, and environmental needs.
  • Develop Care Plans: Develop and implement individualized care plans to meet the unique needs of each patient.
  • Provide Emotional Support: Offer emotional support and guidance to patients and their families during a difficult time.
  • Collaborate with Interdisciplinary Team: Work closely with the interdisciplinary team to ensure seamless care coordination and communication.
  • Document Patient Information: Accurately document patient information, including progress notes, medication lists, and care plans.
Requirements
  • Current RN Licensure: Current RN licensure in the state of practice.
  • Clinical Experience: One year of clinical experience in hospice care or a related field.
  • CPR Certification: Current CPR certification.
  • Reliable Transportation: Access to a dependable vehicle or public transportation.
