Job Summary

As a Registered Nurse Case Manager at Geisinger Health, you will play a critical role in optimizing patient health status by assessing, planning, implementing, coordinating, monitoring, and evaluating all options and services. Your goal will be to manage utilization and practice metrics to refine the delivery of care and maximize clinical, quality, and fiscal outcomes.

Key Responsibilities
  • Integrate evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics into treatment plans that are patient-centric, promoting quality and efficiency in healthcare delivery.
  • Develop systems of care that monitor progress and promote early intervention in acute care situations.
  • Assist in designing, implementing, and evaluating the advanced patient-centered care model.
  • Assess the healthcare, educational, and psychosocial needs of patients or members.
  • Design an individualized plan of care and foster a team approach by working collaboratively with the patient or member, family, primary care provider, and other healthcare team members to ensure coordination of services.
  • Continuously evaluate laboratory results, diagnostic tests, utilization patterns, and other metrics to monitor quality and efficiency results for assigned populations.
  • Work to appropriately apply benefits and utilization management, serving as a resource to the patient or member and healthcare team.
  • Maintain required documentation for all case management activities.
  • Collect required data and utilize it to adjust the treatment plan when indicated.
Position Details
  • Hours: 8 am to 4:30 pm
  • Travel: Less than 25%
  • On-Call: Limited
  • Required Experience: 3 years of RN work experience
  • Required Certification: CCM certification is required in 3 years. Geisinger will assist with this.
