Job Title: Physical Therapist

Centra Healthcare Allied is seeking a skilled Physical Therapist to join their team for a contract assignment in a Skilled Nursing Facility (SNF) / Long Term Care (LTC) setting.

Job Summary:

The Physical Therapist will be responsible for assessing patients' medical conditions, functional capabilities, limitations, and restrictions, and developing a treatment plan to improve their physical function and quality of life.

Responsibilities:
  • Assess patients' needs and develop a plan of care to meet their physical therapy goals.
  • Provide skilled physical therapy services, including interventions and exercises, to patients in accordance with physician orders.
  • Collaborate with other healthcare professionals to plan and evaluate team goals for each patient.
  • Document and record patients' progress and outcomes.
Requirements:
  • Graduation from an accredited Physical Therapy program.
  • Current state license as a Registered Physical Therapist or in the process of obtaining licensure.
  • Current CPR certification.
