Clinical Assessor

Acentra Health is seeking a skilled Clinical Assessor to join our team. As a Clinical Assessor, you will be responsible for conducting assessments to determine the level of care required by individuals in need of personal care services. You will work with clients in their homes or in adult care or supervised living homes to assess their needs and develop personalized care plans.

• Conduct assessments to determine the level of care required by individuals in need of personal care services
• Develop personalized care plans based on assessment results
• Work with clients, family members, and caregivers to ensure that care plans are implemented effectively
• Collaborate with healthcare professionals and other stakeholders to ensure that care plans are aligned with client needs
• Conduct regular reviews of care plans to ensure that they remain effective and relevant

• Registered Nurse or LCSW license by the state of North Carolina
• 2+ years of experience in home care setting preferred
• Experience with community-based individuals needing personal assistance with ADL and IADL tasks is highly preferred
• Experience conducting PCS assessments highly preferred
