Job Summary

Family & Children's Services is seeking a highly motivated and compassionate Case Manager to join our team. As a Case Manager, you will work collaboratively with a multidisciplinary team to provide outreach, engagement, and follow-up service navigation for individuals assessed by the Alternative Response Team (ART). Your primary goal will be to provide client advocacy and assist individuals in accessing all levels of service delivery and community resources they need to recover.

Responsibilities
  • Provide crisis services that divert individuals experiencing a mental health crisis from inpatient hospitalization, jail, homelessness, or emergency departments.
  • Collaborate with multidisciplinary teams to provide a coordinated effort to enhance client recovery.
  • Establish supportive, trusting relationships with individuals with mental illnesses and/or substance abuse issues.
  • Respect client rights and personal preferences in treatment.
  • Utilize the Certified Community Behavioral Health Clinic (CCBHC) model of care to provide whole-person care for adults and children with complex mental health and substance use challenges.
Requirements
  • Bachelor's degree in the social sciences field (required).
  • Case Management Certification or obtain by taking the first available Case Management Certification course provided by ODMHSAS after hire (required).
  • Valid Oklahoma Driver License and satisfactory driving record (required).
