Senior Healthcare Coordinator

We are seeking a highly skilled and dedicated Senior Healthcare Coordinator to join our team at Elevance Health. In this critical role, you will be responsible for managing the care of members with chronic illnesses, co-morbidities, and disabilities, ensuring that they receive the best possible healthcare services.
About the RoleAs a Senior Healthcare Coordinator, you will work closely with a multidisciplinary team of healthcare professionals to develop and implement comprehensive care plans that meet the unique needs of each member. Your expertise and experience will enable you to effectively communicate with members, their families, caregivers, and healthcare providers to ensure seamless coordination of care.
Key Responsibilities- Develop and implement individualized care plans that address the physical, behavioral, and social needs of members.
- Conduct thorough assessments to identify members who require high-risk interventions and coordinate care accordingly.
- Collaborate with healthcare providers, community resources, and other stakeholders to ensure timely access to necessary services.
- Monitor and evaluate the effectiveness of care plans, making adjustments as needed to optimize health outcomes.
- Supervise and direct non-RN clinicians in coordinating services for members, providing guidance and feedback as required.
To succeed in this role, you will need:
- A minimum of 3 years of experience in a Service Coordinator, Case Management, or similar role.
- An active, unrestricted RN license in the applicable state(s).
- Excellent communication and interpersonal skills, with the ability to build strong relationships with diverse stakeholders.
- Strong analytical and problem-solving skills, with the ability to prioritize multiple tasks and manage competing demands.
We also welcome applications from candidates with:
- Bilingual (English/Spanish) skills.
- Experience working with older adults in care management, provider, or other capacities.
- Experience managing a community-based care management caseload.
Elevance Health offers a competitive salary range of $80,000 - $110,000 per year, depending on experience, plus a comprehensive benefits package, including medical, dental, vision, short-term disability, long-term disability, life insurance, 401(k) + match, stock purchase plan, wellness programs, and financial education resources.
