Job SummaryWe are seeking a highly skilled and compassionate Registered Nurse to join our Ambulatory Care team. As a key member of our primary care clinical team, you will provide high-quality, patient-centered care to our patients, collaborating with providers and support staff to ensure seamless care coordination.
Responsibilities- Provide clinical nursing interventions, including assessment, promotion, maintenance, and restoration of patients' health and wellness.
- Collaborate with primary care providers to ensure efficient and timely communication between providers and specialists.
- Support implementation of clinical care pathways for common primary care conditions and participate actively in quality improvement opportunities.
- Utilize evidence-based ambulatory care nursing interventions during clinical visits, telephone encounters, and electronic communication.
- Provide patient and family education for preventive care, chronic disease management, and medication management.
- Document all patient and family interactions and clinical care in the electronic medical record (EMR).
- Current RN licensure in the state of employment.
- AHA BLS certification.
- 2+ years' experience as a Nurse in a primary care or ambulatory setting.
