We are seeking a highly skilled and compassionate Wound Care Specialist to join our team at SSM Health. As a key member of our healthcare team, you will be responsible for implementing and evaluating our wound and ostomy evidence-based care and prevention program to achieve exceptional clinical outcomes.
Key Responsibilities:- Evaluate patients of all ages who are consulted and/or referred by medical staff, nursing staff, and other healthcare disciplines for wound, ostomy, and continence-related needs.
- Collaborate with healthcare providers to allow for timely assessment and inter-professional interventions.
- Develop, communicate, implement, and document a care plan for patients with wound, ostomy, continence issues, and/or are at high risk for skin breakdown.
- Perform pre-operative stoma site marking for urinary and fecal diversions and conservative sharp instrumental debridement and chemical cautery with provider order.
- Design and implement wound, ostomy, continence-related information and patient education appropriate to the patient/family/caregiver's developmental level, learning needs, learning readiness, and cultural values/beliefs.
- Graduate of an accredited school of nursing or education equivalency for licensing.
- Basic Life Support Healthcare Provider (BLS HCP) - American Heart Association (AHA) certification.
- Registered Professional Nurse (RN) - Illinois Department of Financial and Professional Regulation (IDFPR) or equivalent certification in other states.
