We are seeking a highly skilled Physical Therapist (PT) to join our Home Health team in the Toledo, OH area. As a PT with HealthPRO Heritage, you will have the opportunity to work with patients in their own homes, providing one-on-one care and therapy to help them achieve optimal health and functionality.

Responsibilities
  • Evaluate patients' physical conditions, including mobility, strength, and flexibility, using strong clinical assessment and diagnostic skills.
  • Develop and implement effective treatment plans to enhance functionality and alleviate pain, working closely with healthcare providers to ensure comprehensive care.
  • Maintain accurate records of evaluations, progress, and treatment plans, adhering to legal and ethical standards.
Qualifications
  • Degree in Physical Therapy from an accredited institution.
  • Valid state licensure as a Physical Therapist, or license eligible.
  • Strong communication and interpersonal skills, with a patient-centered approach.
  • Ability to lift and assist patients as needed, with physical stamina for extended periods of standing and therapeutic tasks.
