We are seeking a skilled Licensed Practical Nurse to join our team at Crawford Thomas Recruiting. As a LPN, you will play a vital role in delivering high-quality home nursing care to patients, working under the supervision of the Nursing Supervisor and in collaboration with the RN and physician.
Key Responsibilities- Assess patient needs and contribute to the plan of care, ensuring timely and effective delivery of services.
- Perform skilled visits according to the care plan established by the RN and physician, utilizing objective observational skills and demonstrating proper judgment and initiative.
- Document patient information accurately and concisely in the home care record, adhering to agency policies and procedures.
- Collaborate with the primary RN to coordinate patient care needs and contribute to case conferences to enhance the team approach to patient care.
- Stay up-to-date with current medical/surgical treatment modalities and assist the nursing supervisor and RN in the intake process of patient referrals.
- Assist in office operations, maintain the home care record, and acquire continuing education/contract hours to maintain current licensure.
- A graduate of an approved school of professional nursing, licensed as a Licensed Practical Nurse by the state of Ohio.
- Two years clinical nursing or one year home health care experience.
- Effective verbal and written communication skills, with the ability to organize and manage time effectively.
- Computer knowledge and skills, with a valid Ohio driver's license and properly insured transportation.
