Job Summary

Sweetser is seeking a skilled Case Manager to join our Transitional Aged Youth program in Saco. As a key member of our team, you will provide essential support and services to young individuals in need, helping them navigate the healthcare system and achieve their goals.

Key Responsibilities
  • Conduct thorough assessments of participants' physical, mental, and social service needs, working closely with guardians and natural supports.
  • Provide critical information on primary and emergency care, assisting in the exploration of less restrictive alternatives to hospitalization.
  • Refer participants to relevant services, including behavioral health and medical services, housing, food, clothing, and other basic needs.
  • Promote participant wellness and develop individualized service plans in conjunction with the person served.
  • Document services accurately and efficiently, adhering to organization, state, and federal documentation requirements.
  • Participate in crisis intervention, resolution, and follow-up services, identifying and documenting unmet participant needs.
  • Facilitate active and timely communication among team members and facilitate transportation to medical appointments and community activities.
Requirements
  • Bachelor's degree in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing, or a closely related field.
  • Maintain a valid state driver's license and be certified in Mental Health Rehabilitation Technician-Certified (MHRT-C) or eligible.
  • Prior experience working in or with medical practices is preferred, and proficiency in electronic health records is essential.
