Job Summary

We are seeking a highly skilled and experienced LVN Charge Nurse to join our team at Regency Integrated Health Services. As a Charge Nurse, you will play a vital role in ensuring the provision of quality care to our residents, promoting their physical, mental, and psychological well-being.

Key Responsibilities
  • Assess and Document Resident Condition: Observe, assess, document, and report resident condition/changes during rounds.
  • Implement Care Plans: Assure implementation of care plans as directed by physicians and nursing staff.
  • Medication Administration: Administer medications as prescribed by physicians.
  • Collaborate with Physicians: Conduct rounds with physicians and record visits in clinical records.
  • Narcotic Reconciliation: Perform narcotic reconciliation with incoming and outgoing shifts.
  • Identify and Address Resident Changes: Identify significant changes in the condition of residents and take necessary action per facility policies and procedures.
  • Oversee Aides: Oversees and monitors aides to ensure quality care is provided.
  • Incident Reporting: Initiate and complete Incident and Accident Reports per facility policy.
  • Discharge Planning: Discharge residents per facility policy.
  • Nursing Assessment: Complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition.
  • Communication: Communicate with resident's point of contact when they ask for a status update or if there is a change in condition.
Requirements
  • Education: Has completed an LVN program at an accredited vocational school or college.
  • Licensure: Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
  • Experience: Experience working in a skilled nursing facility is preferred.
