Job Summary


We are seeking a skilled Private Duty Nurse to provide high-quality, patient-centered care to medically fragile patients in the comfort of their own homes.


Key Responsibilities



  • Deliver skilled nursing care to patients with complex medical needs
  • Develop and implement personalized care plans to meet the unique needs of each patient
  • Collaborate with healthcare teams to ensure seamless care coordination
  • Provide education and support to patients and their families

Requirements



  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) with current licensure
  • Experience working with medically fragile patients
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
