Job Title: Home Health

At Parentis Health, we are seeking a skilled and compassionate Home Health Nurse to join our team. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and promoting health and well-being in the comfort of patients' homes.

Responsibilities:
  • Conduct comprehensive health and psychosocial evaluations to identify patients' needs and develop personalized care plans.
  • Monitor patients' conditions, provide health education, and coordinate services to ensure seamless care.
  • Evaluate the effectiveness of nursing services and make adjustments as needed.
  • Prepare and present patient records to the Clinical Record Review Committee.
  • Collaborate with attending physicians, supervisors, and other healthcare professionals to ensure optimal patient outcomes.
  • Participate in case conferences, staff development meetings, and other educational activities to stay up-to-date on best practices.
  • Supervise and evaluate the care provided by Home Health Aides, as needed.
Qualifications:
  • Current licensure as a Registered Nurse in the state of California.
  • One or more years of experience in community/home health agency or hospital setting, preferred.
  • Strong knowledge of home healthcare principles and techniques, as well as required documentation.
