We are seeking a compassionate and skilled Palliative Care Advanced Practice Professional to join our team at Four Seasons, The Care You Trust.

About the Role

This is an exciting opportunity for a dedicated healthcare professional to work under the direction of a physician to provide patient care consultation and treatment to patients with serious illnesses. As a Palliative Care Advanced Practice Professional, you will be an integral part of our interdisciplinary team, working collaboratively to achieve optimal patient outcomes.

Responsibilities
  • Provide direct client care consultations, including participation in patient and family counseling and management of symptoms related to life-limiting conditions.
  • Take clinical call for palliative care/hospice clients and serve as attending for hospice clients when necessary.
  • Actively participate in Interdisciplinary Team meetings for development and evaluation of the clients plan of care.
  • Create and maintain accurate patient records.
  • Teach and mentor nurses and other staff to obtain quality care outcomes efficiently with excellent patient and family experience.
Requirements
  • Master's degree and clinical rotation preferred.
  • Two years of post-education experience required.
  • Palliative care experience is highly desirable.
  • Current NP or PA Board Approved Certification.
  • Current NC Medical License in good standing.
  • Current DEA Registration in good standing.
