Job Summary

We are seeking a compassionate and motivated Physical Therapist to join our team. As a Physical Therapist at JAG, you will be responsible for assessing patients' strengths and weaknesses, devising creative and effective treatment plans, and implementing a plan of care that promotes patient success.

Key Responsibilities
  • Collect and record patient care data, including initial evaluations, daily notes, reassessments, and discharge plans.
  • Prepare for evaluations by obtaining patient history, researching unfamiliar diagnoses, and selecting and administering tests and measures.
  • Formulate diagnoses, goals, and plans of care based on evaluation results and best practice.
  • Establish treatment plans, collaborate with interdisciplinary teams, and assess the need for modifications to plans of care and goals.
  • Provide education and training to patients and their support systems.
  • Oversee support staff, including PTA, ATC, and PT aides.
Requirements
  • Bachelor's or Master's Degree in Physical Therapy required; Doctorate degree preferred.
  • Licensed Physical Therapist in the state practicing in.
  • Completed state-regulated CEU requirements in the appropriate state (NY, NJ).
  • Valid CPR license.
  • Experience/knowledge with EMR software preferred.
  • Excellent oral and written communication skills.
  • Strong clinical decision-making skills.
  • Excellent work ethic and dedication to patient success.
