Job Summary

We are seeking a compassionate and skilled Home Health Registered Nurse to join our team. As a Home Health Registered Nurse, you will be responsible for providing high-quality patient care, collaborating with our interdisciplinary team, and promoting a positive patient experience.

Key Responsibilities

  • Provide direct patient care, including assessments, medication management, and wound care
  • Develop and implement individualized care plans that meet the unique needs of each patient
  • Collaborate with our interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care
  • Communicate effectively with patients, families, and caregivers to ensure a positive patient experience
  • Participate in ongoing education and training to stay up-to-date on best practices and industry developments

Requirements

  • RN license in the state of employment
  • Graduate from an accredited school of nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
