Key Responsibilities:- Conduct regular wellness visits for all residents to assess their health and wellness needs.
- Develop and implement individualized care plans to meet the unique needs of each resident.
- Collaborate with the Resident Care Director and community team to ensure high-quality care and services are delivered.
- Monitor and report any changes in resident health status to the Resident Care Director and attending physicians.
- Provide education and support to residents and their families on health and wellness topics.
- Graduate of an approved college or school of nursing.
- Current state license as a professional Registered Nurse (RN).
- Minimum of one year experience in home health, assisted living, hospital, or long-term care environment.
- Knowledge of nursing practices, techniques, and methods applied to health and wellness resident care.
- Ability to work weekends, evenings, and flexible hours to meet the needs of our residents.
