Key Responsibilities

Assume comprehensive nursing responsibility for implementing the Plan of Care for each hospice patient. Regularly assess personal and professional performance to enhance productivity and the quality of care provided.
  • Conduct the initial nursing assessment to determine eligibility for hospice services during a visit within forty-eight (48) hours of referral.
  • Identify the physical, psychosocial, and environmental needs of the patient and family, reassessing as necessary, at least every fifteen (15) days.
  • Initiate and coordinate the plan of care.
  • Document issues, relevant goals, interventions, and responses from the patient and family regarding hospice care.
  • Collaborate daily with the patient, family, attending physician, and other members of the interdisciplinary team to provide comprehensive care.
  • Educate and guide the patient and family in self-care techniques when appropriate.
  • Maintain accurate and pertinent clinical documentation regarding the patient's condition.

Qualifications and Experience

Experience Requirements
  • A minimum of one year of clinical experience.

License Requirements
  • Current RN licensure in the state of practice and one year of clinical experience.
  • Current CPR Certification.
  • Valid driver's license, vehicle insurance, and access to reliable transportation or public transit.

Additional State-Specific Requirements

  • LA: A minimum of two years of full-time experience as an RN is required. However, two years of full-time clinical experience in hospice care as a licensed practical nurse may substitute for the required two years of experience as a registered nurse.
