ROLE AND RESPONSIBILITIES:


The Registered Nurse (CAP RN 1) position serves as an entry-level role within the Clinical Advancement Program for Registered Nurses.

The CAP RN 1 is responsible for delivering and coordinating high-quality patient care, which includes assessment, planning, intervention, and evaluation for a designated group of patients.


As a vital member of the Kidney Disease healthcare team, the CAP RN 1 engages in decision-making, educational initiatives, leadership roles, and quality improvement efforts aimed at enhancing patient care outcomes and operational efficiency.


KEY DUTIES:


All responsibilities must align with the policies and standards of the organization and relevant regulations.


Perform essential functions under the supervision of the Program Manager and with support from the Educator or more experienced CAP RNs during the initial employment period.


Expected to fulfill the requirements for advancement to the next CAP level within 24 months of hire.


Conduct ongoing collection and analysis of patient data throughout treatment phases, documenting findings accurately in the medical record, and adjusting treatment plans as necessary while notifying relevant team members.


Identify and report variations in patient status to appropriate healthcare team members for collaborative input.

Assess daily patient care needs and work collaboratively with both direct and ancillary care staff.

Deliver safe and effective patient care as prescribed for all treatment modalities.


Facilitate communication with both internal and external dialysis providers and hospital personnel to ensure continuity of care.

Administer medications as prescribed and in accordance with established agreements.

Ensure nephrology physician orders are accurately recorded in the medical record.

Thoroughly document all treatment data in a timely manner.


Implement, monitor, and document patient responses to prescribed treatments, including transfusions, while notifying the physician of any adverse reactions.


Familiarize yourself with emergency policies and procedures to assist in emergency situations.

Delegate appropriate tasks to support staff, ensuring adherence to policies and procedures.

Promote infection control and maintain a safe environment.

Assess and document patient and family educational needs to provide ongoing education.

Instruct facility staff on specialized renal care procedures.

Collaborate with the interdisciplinary team to update nursing assessments and care plans.

Initiate and document quality assurance activities as directed.

Serve as a resource for the healthcare team and participate in training for new staff as assigned.

Attend all required staff meetings.

May be assigned to assist in outpatient facilities as needed.

Support and promote the organization's mission, vision, and values.

Perform additional duties as assigned within regulatory standards.

The employee may occasionally need to move equipment weighing up to 200 lbs. and lift solutions weighing up to 30 lbs.

The work environment is typical of a healthcare facility with controlled air temperature and moderate noise levels. Exposure to infectious materials may occur.


Travel between assigned facilities and participation in on-call rotation may be required.



SUPERVISION:
Oversight of Patient Care Technicians/LPNs may be assigned after meeting specific training and experience requirements.

EDUCATION:
Graduate of an accredited nursing program.

Current state licensure is required.

EXPERIENCE AND REQUIRED SKILLS:


Entry-level position for RNs with less than 2 years of nephrology nursing experience or RNs with a BSN and less than 1 year of nephrology experience.

Minimum of 9 months of RN experience is preferred.

6 months of nephrology experience is required before independent practice.

Experience in hemodialysis or ICU is preferred.

Successful completion of required training and certifications is mandatory.

Must meet all employment conditions, including color vision testing.

Participation in ongoing training and education is expected.
