Job Summary

At HealthPRO Heritage, we are seeking a highly skilled Speech-Language Pathologist to join our team. As a Speech-Language Pathologist, you will play a vital role in providing and directing speech therapy services to patients, helping them reach their maximum performance potential within a medical model of practice.

Key Responsibilities
  • Evaluation and Assessment: Conduct thorough assessments of patients to determine and diagnose communicative deficits and equipment needs.
  • Treatment Planning: Develop and implement individualized treatment programs based on initial evaluations and containing achievable functional goals.
  • Documentation and Record-Keeping: Maintain timely and accurate departmental records, including progress notes and discharge summaries.
  • Team Collaboration: Work with nursing staff, residents, and families to maximize residents' use of functional abilities.
  • Education and Training: Instruct residents and family members on therapeutic procedures to be continued at home following discharge.
  • Continuing Education: Maintain current knowledge of community resources and participate in continuing education opportunities to stay up-to-date with clinical best practices.
  • Quality Improvement: Participate in quality improvement processes, including responding to results of medical record audits and resident satisfaction surveys.
  • Professional Development: Adhere to facility policies and procedures, participate in facility quality improvement and safety programs, and demonstrate a knowledge of OBRA regulations, HIPAA, and third-party reimbursement guidelines.
Requirements
  • Education: Master's Degree in Speech and Language Therapy.
  • Certification: Hold a current Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) credential offered by the American Speech-Language-Hearing Association.
  • Licensure: Must maintain valid state Speech Language Pathology licensure.
