Primary Care Nurse Practitioner

A leading health services network in New York City is seeking a licensed Nurse Practitioner to join their team at one of their primary care centers in Queens. As a key member of the healthcare team, the Nurse Practitioner will be responsible for delivering high-quality patient care and contributing to the development of clinical protocols.
Key Responsibilities:- Conduct thorough patient assessments, develop and implement diagnostic and treatment plans, and provide patient education and counseling on preventive care and chronic disease management.
- Collaborate with healthcare teams to discuss patient care and implement new clinical protocols, ensuring seamless communication and coordination.
- Deliver high-quality care, ensuring patients are informed about post-discharge and follow-up instructions, and complete patient charts in a timely manner to ensure compliance with medical standards and billing requirements.
- Graduate of an accredited Nurse Practitioner program, preferably as a Family Nurse Practitioner (FNP).
- Active NYS Nurse Practitioner (NP) license and strong skills in patient assessment, clinical care, and thorough documentation.
- Patient-focused with a commitment to delivering compassionate, high-quality care, and excellent interpersonal and communication skills (both written and verbal).
- Detail-oriented and able to manage multiple priorities in a fast-paced clinical setting, with the ability to maintain accurate and timely medical records.
- 3+ years of experience in an outpatient primary care setting and graduate of a NYS Registered Nurse Practitioner Program.
