Job Title: Home Care Nurse

About the Role:

Aveanna Healthcare is seeking a skilled and compassionate Home Care Nurse to provide high-quality care to adult patients in the Hudson, MA area. As a Home Care Nurse, you will work one-on-one with patients, providing personalized care and support to help them achieve their health goals.

Responsibilities:

  • Implement and manage patient care plans in collaboration with physicians and other healthcare professionals.
  • Conduct thorough patient assessments and develop individualized care plans to meet their unique needs.
  • Administer medications, treatments, and therapies as prescribed by physicians.
  • Document patient progress and care provided in a timely and accurate manner.
  • Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies.

Requirements:

  • Current, unrestricted RN license in the state of Massachusetts.
  • Minimum 6 months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience preferred.
