Liberty Health is seeking an experienced Hospice Clinical Manager to join our team. As a Hospice Clinical Manager, you will be responsible for coordinating case management of all patient services, supervising assigned care staff, and assisting the administrator as necessary.
Key Responsibilities:
- Case Management: Coordinate the delivery of patient care services, ensuring that patients receive high-quality, compassionate care.
- Staff Supervision: Supervise and mentor care staff, providing guidance and support to ensure that they are equipped to provide excellent patient care.
- Documentation: Maintain accurate and timely documentation of patient care and staff performance.
- Collaboration: Work closely with the administrator to ensure that the hospice program is running smoothly and efficiently.
- Quality Improvement: Identify areas for quality improvement and implement changes to enhance patient care and staff performance.
Requirements:
- Education: Current RN licensure in North Carolina and successful completion of nursing education from an approved school of nursing.
- Experience: Minimum three (3) years of clinical nursing experience, with a focus on hospice care.
- Skills: Strong critical thinking and communication skills, with the ability to work independently and as part of a team.
- Certifications: CPR certification.
