Key Responsibilities:

  • Conduct comprehensive social work assessments to identify beneficiaries' needs and develop personalized plans to improve their health and well-being.
  • Collaborate with community agencies and healthcare providers to ensure seamless coordination of services and resources.
  • Develop and implement innovative programs to promote health education, disease prevention, and health promotion among beneficiaries.
  • Network with vendors to develop health fairs and events that enhance beneficiary access and participation in health services.
  • Conduct educational programs for beneficiaries in collaboration with an interdisciplinary team and local partners.
  • Identify and address issues affecting beneficiaries, implementing case conferences with medical directors and/or vendors to enhance, maintain, or improve quality of life.
  • Develop and maintain effective working relationships with field health and medical management staff, beneficiaries, their families, and community agencies.
  • Prepare written reports and/or oral presentations to trustees and/or operations staff on the status of activities in the clinical field service office.
  • Record all beneficiary-related interactions timely in the Fund's designated medical management system.

Requirements:

  • Education: Bachelor of Science in Social Work (BSW) or comparable undergraduate degree required; Master of Social Work (MSW) preferred.
  • Licensure Requirement: LSW in both Ohio and West Virginia.
  • Minimum of 5 years of relevant experience and must have knowledge in health issues related to geriatrics and experience in health education.
  • Must have excellent time management skills to balance the multiple complexities of the position.
  • Able to constantly travel within the geographic area services with occasional overnight travel.
  • Candidate must reside in Belmont County, OH or a closely neighboring county.
