Centra Healthcare Solutions is seeking an experienced Physical Therapist to work in the specialty area of SNF / LTC.

This is a direct hire / permanent placement position that requires the candidate to be licensed or in the immediate process of obtaining Physical Therapist licensure in the state of WI.

Responsibilities:
  • Provide physical therapy services to patients in a skilled nursing facility or long-term care setting.
  • Develop and implement individualized treatment plans to meet the unique needs of each patient.
  • Collaborate with other healthcare professionals to ensure comprehensive care.

Requirements:
